This program aims to support and assist our community’s caregivers. By providing care management services, financial assistance, and the opportunity for respite; we hope to help prevent caregiver burn-out and strengthen the caregiving relationship.
Applicants
The Caregiver Support Program recognizes various care-giving arrangements including grandparents raising grandchildren and older relative caregivers of adults with a disability. The caregivers in these examples must be age 55 or older. Also considered are caregivers over the age of 18 caring for an individual 60+ years. Program eligibility is determined by a comprehensive needs assessment, the care receiver’s household income, and available funding.
Financial Assistance
Financial assistance may be available in the form of monthly reimbursement for care-giving expenses such as supplies and services. The program also recognizes that larger purchases may be necessary in order to maintain accessibility and safety for both the caregiver and care receiver. For this reason, funds are set aside to help offset the cost of home modifications and assistive devices.
Care Management
Each caregiver will be assigned a care manager who has been trained to recognize areas of need, customize care plans accordingly, and share information on helpful community resources and training opportunities. The care management team works closely with our caregivers, care receivers, and their families- promoting both advocacy and empowerment.
