Family Caregiver Support Program

This program is designed to assist families or others who are providing care for incapacitated or Caring for you so you can better care for your loved one.functionally dependent older adults who reside at home. The focus of this program is to address the needs of the “caregiver” by empowering the caregiver and care receiver to choose the services or assistance which they feel are most needed in order to reduce stress. 

Financial Assistance

Limited financial assistance is available to reimburse the caregiver for the cost of needed supplies for the care receiver or for the purchase of services such as respite care or adult day care. A one time grant is also available (contingent on availability of funding) to pay for home modifications or assistive devices. Benefit counseling and/or assistance may also be provided to acquaint caregivers with available services and resources, as well as counseling or training to help reduce stress to the caregiver by improving care giving skills.

Other Funding

Limited funding is also available to assist eligible grandparents over the age of 60 who are the primary caregivers of grandchildren aged 18 or younger who live in the same home and who have functional disabilities or developmental delays.

Available funding for participation in this program is extremely limited and is based on the level of functional dependence of the care receiver and on income. An Aging Care Manager is assigned to assess the needs of the caregiver, to evaluate the needs of the care receiver and to review the required financial documentation to determine eligibility.
